Breakthrough Functional Medicine, PLLC is a cash-pay, membership functional medicine practice here in Wilmington, NC. We do not do rushed, seven-minute visits or hand someone a prescription and send them on their way. We do the deeper work of getting to the root: real time with our members, comprehensive testing, food-first and lifestyle-first plans, and care for the whole person, body, mind, and spirit. We believe the body is intricately connected, and we treat it that way, never as a list of symptoms to manage.

So many of our members find us worn down and unheard, after being told their labs look "normal" while they know in their gut that something is off. They're walking through hormonal imbalance, gut struggles, thyroid and metabolic issues, autoimmunity, mold and CIRS, and complicated stories conventional medicine hasn't been able to untangle. We get to walk alongside them, pairing thoughtful clinical care with in-office recovery tools like our HOCATT, PEMF mat, and infrared sauna, along with health coaching and a warm community that roots for one another.

How Our Membership Works

We're a cash-pay membership practice, so we don't bill insurance. Members pay a simple monthly membership for direct, ongoing access to their provider and a set number of visits across the year, with a few tiers depending on how much support someone needs. Everyone starts with a comprehensive Foundational Assessment, a deep-dive intake plus advanced testing that sets the whole root-cause plan in motion. From there, membership includes their ongoing visits, health coaching, and access to our in-office modalities. Because we're free of insurance codes and rushed appointments, we get to spend real time with people and build care around what they actually need.

About The Role

You'll come alongside me as BFM's second provider, sharing a panel of members in the same root-cause, whole-person way we care for everyone here. This is a shared-care role. I conduct each member's foundational assessment and their key milestone visits, and you'll carry the follow-up visits in between, reviewing progress, fine-tuning protocols, and keeping members moving toward the healing they came here for. As you settle in and earn the trust of the panel, your scope grows right along with you.

We'll build your schedule together as we grow, working toward patients 3 to 4 days a week. We're a small team, so you'll help us grow too, through member education and a real, ongoing hand in helping us reach the people who need us (more on that below). And you won't be doing any of it alone. You'll have a clinical coordinator, a health coach, and operations support in your corner.

Core Responsibilities

  • Complete a structured onboarding, then carry a shared panel of members, conducting follow-up visits and managing ongoing, personalized care (with mentor support as you get up to speed)

  • Interpret functional and conventional labs and build root-cause, food-first protocols consistent with our clinical approach

  • Sequence care thoughtfully (foundations and adrenals before hormones; systems before symptoms)

  • Manage prescriptive care under our collaborative practice agreement

  • Respond to member portal messages within our service standards

  • Contribute to member education, co-leading a workshop or community talk and contributing to newsletter or educational content

  • Partner with the clinical coordinator and health coach on labs, follow-up, and member experience-

  • Other duties as reasonably assigned to support a small, growing practice

Who We're Looking For

I'm open to both experienced providers and those newer to practice. Experience in functional medicine is welcome, but it isn't a requirement. Sometimes someone fresh brings the curiosity and teachability that help them grow into our model from the ground up. For the right early-career candidate, I'll invest in your training (funded AFMCP plus gut and hormone modules plus incubator access) in exchange for a multi-year commitment, and mentor you closely as you grow. What matters most for any candidate is a heart for root-cause care and a willingness to learn the pillars our members rely on. I'm looking for someone warm, coachable, trusted by patients, and glad to be part of a small practice's growth.

What You'll Need

  • An active, unrestricted North Carolina APRN license (FNP-BC or equivalent), or the ability to obtain one before your start date

  • Current national board certification through ANCC or AANP

  • Prescriptive authority and willingness to practice under a collaborative practice agreement

  • Real skin in the game in functional and integrative medicine, whether through clinical experience, completed coursework (IFM AFMCP, A4M modules, an integrative fellowship), or self-directed study paired with a real desire to go deeper.   • Comfort in a small, entrepreneurial practice where roles flex and everyone pitches in

Nice to Have (we'll train the right person)

  • Institute for Functional Medicine (IFM) certification, or interest in pursuing it

  • Experience with any of our clinical pillars: hormone/HRT, gut, adrenal, thyroid, metabolic, autoimmunity, and mold/CIRS

  • Familiarity with cash-pay membership, DPC, or concierge models

  • Interest in nutrigenomics and genetic pathway analysis

  • Experience with Practice Better or a similar EHR/portal

If You're Earlier in Your Career

  • A foundation in functional medicine, such as completing AFMCP or documented self-study, along with a drive to learn quickly

  • Willingness to complete our funded training and grow with us over the next several years
